	Access privileges are modified and revoked whenever a user’s job function or access needs change or whenever a user is no longer employed. This revocation occurs on the effective date of the user’s end of employment or sooner if warranted by circumstances.

	Visitors are required to sign a log that records the date, time of arrival and departure along with their name, organization, purpose for visit and phone number.
	Visitors must be escorted as appropriate and, if working near or with ePHI, have appropriate authorization and supervision.
	All workstations require a User ID and password with 3-trial log-in set up.
	Cable locks are used on all computers to prevent theft.
	All workstations have an automatic log-off set up.

Technical access controls

      Technical access controls are implemented through the use of unique user ID, emergency access procedure, and automatic log-off.

Person or entity authentication

      A unique User ID is assigned to staff members who are authorized to access the agency’s information system and ePHI.

Enforcement

      The Security Rule is enforced by the Center for Medicare and Medicaid Services (CMS) not by the Office of Civil Rights like the Privacy Rule.

Personal safety guidelines…

·          When calling patients from your home, use *67 to block out your telephone number; patients may have caller ID.

·          Be aware of you concerns: If you have a feeling that a situation is dangerous, it probably is.

·          Do not remain in a parked car for a prolonged period before or after a visit.

·          Dress appropriately following the dress code. Wear shoes that fit comfortably and well so that you can move quickly and safely.
